About Evoke Strong PreventT2 Lifestyle Change Program:
Research shows modest behavior changes such as making better food choices, engaging in physical activity, and managing stress prevents or delays the development of type 2 diabetes.
Evoke Strong PreventT2 lifestyle change program is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). The program uses the PreventT2 curriculum developed by the CDC to guide participants in making healthy lifestyle changes.
Who is this program for?
One in three American adults has prediabetes. Without lifestyle changes, 15 to 30 percent of people with prediabetes can develop type 2 diabetes within five years. You are eligible to enroll in the program if you are:
The Benefits of the Program:
As part of the lifestyle change group, you will be connecting with other participants and your lifestyle coach to learn the skills you need to make lasting lifestyle changes.
Being part of a group gives you the support you need from those facing similar challenges and trying to make changes as you are. You get to share your experiences and learn from one another. Together you celebrate successes as well as discover ways to overcome obstacles.
What to Expect:
- It takes time to learn and adopt sustainable healthy habits. Therefore, this program runs for ONE YEAR. Our group meets weekly for the first 16 weeks, followed by bi-weekly and monthly sessions for the remaining months. By staying in the program, you will notice the small, healthful changes you make multiply and transform you from the inside out.
- In every session, you will be interacting with fellow participants as you discuss various lifestyle topics. A few examples are making healthy food choices, becoming more active, healthy stress relief strategies, and understanding your triggers. Participants find value in learning and applying the skills but, most importantly, the support they receive from one another.
