About Evoke Strong PreventT2 Lifestyle Change Program:
Research shows modest behavior changes such as making better food choices, engaging in physical activity, and managing stress prevents or delays the development of type 2 diabetes.
Evoke Strong PreventT2 lifestyle change program is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). The program uses the PreventT2 curriculum developed by the CDC to guide you in taking small steps that add up to a healthy lifestyle you can stick with and enjoy!
Who is this program for?
One in three American adults has prediabetes. Without lifestyle changes, 15 to 30 percent of people with prediabetes can develop type 2 diabetes within five years. You are eligible to enroll in the program if you are:
The Benefits of the Program:
As part of the lifestyle change group, you will be connecting with other participants and your lifestyle coach to learn the skills you need to make lasting lifestyle changes.
Being part of a group gives you the support you need from those facing similar challenges and trying to make changes as you are. You get to share your experiences and learn from one another. Together you celebrate successes as well as discover ways to overcome obstacles.
What to Expect:
It takes time to learn and adopt sustainable healthy habits. Therefore, this program runs for ONE YEAR. Our group will meet every week for the first 16 weeks, followed by bi-weekly and monthly sessions for the remaining months. By attending most sessions, you receive the encouragement and support you need to make small, healthful changes. Over time, you’ll notice these changes multiply and transform you from the inside out.
Each session is designed to be interactive as we talk about various healthy lifestyle topics. A few examples are eating well, becoming more active, learning how to cope with stress, and understanding your triggers. You will find value in learning and applying the skills but, most importantly, the support you receive from one another.
